Case Managers - Immediate Interviews Bronx and Brooklyn

Pay: $26 - 28/hr

What You’ll Do:

• Conduct client interviews and assessments using risk assessment tools to determine eligibility and complete new client intakes.
• Partner with clients to create personalized service plans that support their short- and long-term goals.
• Connect individuals and families to vital resources such as housing programs, public benefits, employment services, and legal assistance.
• Work closely with landlords and clients to ensure housing meets habitability standards and help resolve tenancy issues.
• Assist clients with applying for benefits, entitlements, and emergency financial assistance to address urgent housing needs.
• Provide ongoing support through regular follow-ups and progress tracking.
• Maintain accurate, organized case documentation and ensure files remain audit-ready in both paper and electronic systems.
